Troubleshooting Tips
Sometimes respondents have difficulties accessing the P5 Needs Assessment in the first place and sometimes respondents have difficulties in gaining access at later dates. These troubleshooting tips should help. If not please contact Matt Melinyshyn at 613-382-0454, melinysh@kos.net
1. Your username is the email address you provided when you registered.
2. Passwords are case sensitive. If you entered CUBA as your password when you registered, then cuba will not work and vice versa.
3. Cookies have to be enabled on your web browser. This is controlled through Tools in Internet Explorer and or you can contact Information Technology in your hospital. |
